Hearts Filled LLC Mobile Crisis Response Services

Mobile Crisis Response services are available 24 hours per day, seven days per week to provide immediate, on-site intervention for individuals experiencing a behavioral health crisis. Services are delivered in real time at the location where the crisis is occurring, ensuring timely assessment, stabilization, and connection to appropriate supports.

Mobile Crisis Response is designed to support individuals in the following ways:

• Provide rapid, community-based response to behavioral health crises • Deliver on-site assessment, de-escalation, and short-term stabilization services

• Meet individuals in environments where they feel safe and comfortable to promote engagement and cooperation

• Reduce the need for law enforcement involvement, emergency department visits, and psychiatric hospitalization when clinically appropriate

• Coordinate with family members, caregivers, and natural supports when appropriate

• Facilitate referrals and warm handoffs to ongoing behavioral health, medical, and community services

• Support crisis resolution while promoting individual safety, dignity, and recovery

Hearts Filled LLC Community Stabilization Services

Community Stabilization Services provide direct, intensive mental health interventions to non hospitalized individuals who are experiencing an acute behavioral health crisis or period of significant emotional distress. Crises may include, but are not limited to, aggression, self harming behaviors, suicidal ideation, or other behaviors that place the individual or others at risk.

Hearts Filled LLC maintains qualified mental health professionals who are available to respond 24 hours per day, seven days per week, providing mobile, community-based services at the location of the individual. Services are designed to deliver short-term, intensive support to reduce the risk of psychiatric hospitalization, homelessness, or further crisis escalation.

Eligibility for Services

To qualify for Community Stabilization Services, individuals must demonstrate clinical necessity resulting from an acute psychiatric condition that places them at risk of psychiatric Website Descriptions and Content hospitalization or loss of community stability. Individuals must meet at least two (2) of the following criteria at the time of admission:

Guidelines for Admission

1. Experiencing significant difficulty establishing or maintaining interpersonal relationships to the extent that the individual is at risk of psychiatric hospitalization, homelessness, or isolation from social supports;

2. Experiencing impairment in activities of daily living (ADLs), such as personal hygiene, food preparation, adequate nutrition, or financial management, to a degree that health or safety is compromised;

3. Exhibiting behaviors that are so inappropriate or dangerous that immediate intervention by mental health professionals, social services, law enforcement, or the judicial system has been required or is currently necessary;

4. Demonstrating cognitive or judgment impairments that prevent the individual from recognizing personal danger or engaging in appropriate social behavior.

Goals for Treatment

The primary goal of Community Stabilization Services is to provide immediate, structured intervention during a behavioral health crisis, promote safety, and stabilize the individual in the community. Services are designed to:

• Reduce the need for psychiatric hospitalization and emergency department utilization

• Restore the individual’s ability to function safely in their home and community

• Strengthen coping skills and crisis management strategies

• Connect individuals to ongoing behavioral health and community-based supports

Addiction Counseling – ASAM Level 2.5 Partial Hospitalization Program (PHP)

Service Description

The Partial Hospitalization Program (PHP) is a highly structured, intensive outpatient behavioral health service that provides comprehensive, short-term treatment to individuals experiencing acute psychiatric symptoms, substance use disorders, or co-occurring conditions that significantly impair functioning but do not require 24-hour inpatient hospitalization. The Website Descriptions and Content program is designed to stabilize symptoms, reduce substance use, enhance coping skills, and prevent hospitalization or re-hospitalization.

PHP is classified as Level 2.5 of care under the American Society of Addiction Medicine (ASAM) Criteria and is recognized by the Virginia Department of Medical Assistance Services (DMAS) and the Department of Behavioral Health and Developmental Services (DBHDS) as a medically necessary service for individuals with serious mental illness (SMI), serious emotional disturbance (SED), and/or co-occurring disorders. Services are delivered by an interdisciplinary treatment team of licensed and credentialed professionals and are provided up to 20 hours per week, typically Monday through Thursday between 9:30 a.m. and 3:00 p.m.

Admission Criteria (DMAS / DBHDS / ASAM Aligned)

To qualify for PHP services, individuals must meet all of the following criteria:

1. Clinical Necessity

The individual is experiencing acute psychiatric symptoms, substance use–related impairments, or co-occurring conditions that substantially interfere with daily functioning and require active, structured treatment.

Presenting symptoms and behaviors may include, but are not limited to:

• Frequent or intense mood instability, psychotic symptoms, hallucinations, or delusional thinking

• Suicidal ideation without imminent intent or plan requiring inpatient hospitalization

• Significant impairment in occupational, educational, or interpersonal functioning

• Continued substance use despite adverse consequences or recent relapse following prior treatment

2. Level of Care Determination

Based on application of the ASAM Criteria, the individual requires services consistent with Level 2.5 Partial Hospitalization, including one or more of the following:

• Elevated risk or severity in Dimension 1 (Acute Intoxication and/or Withdrawal Potential) and/or Dimension 2 (Biomedical Conditions and Complications) that can be safely managed in a PHP setting

• Moderate to severe impairment in Dimension (Emotional, Behavioral, or Cognitive Website Descriptions and Content Conditions and Complications) requiring intensive therapeutic intervention

• Ongoing risk of clinical deterioration without daily, structured treatment programming

3. Ability to Participate and Remain Safe in the Community

• The individual is able to safely participate in treatment without requiring 24-hour supervision

• The individual has access to adequate natural or formal supports, or can otherwise be safely maintained in the community during non-program hours

• The individual demonstrates the capacity and willingness to engage in therapeutic services and comply with program expectations
